About The Opportunity The Health Care Social Worker reports to the Program Manager and as a member of a multi-disciplinary team, provides a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning and case management. The Health Care Social Worker is actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.
This role is unique in that it enables you to develop lasting relationships with residents and their families and also allow for a more in-depth scope of practice as supporting residents goes beyond resource identification and discharge planning.
About You We would love to hear from you if you have the following:
- Master's Degree in Social Work from a recognized university
- Registered or eligible for registration with the NSCSW
- 2 years of clinical experience as a clinical Social Worker required
- Strong counselling and communication skills
- Experience in Long Term Care will be considered an asset
- Experience in geriatrics considered an asset
- Knowledge and experience in dementia considered an asset
- Capacity to achieve a high degree of autonomous practice
- Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving and continuum of care and intervention skills that come from knowledge of theoretical frameworks/approaches to practice
- Preferred experience working in Geriatrics and with people who have Dementia
- Ability to prioritize effectively
- Ability to work effectively as part of an interdisciplinary team
- Competencies in other languages an asset, French preferred

Please note: consideration may be given to those with a Master of Social Work Education and less than two-year experience in a developmental capacity within a different pay scale.

Compensation And Benefits Healthcare Social Worker (Masters) - $40.18 - $54.14 Hourly
Developmental Social Worker (Masters) (HTH130) - $35.83 - $38.79 Hourly
