Responsibilities

Tasks

  • Consult with other health care professionals to evaluate treatment plans
  • Interview clients, prepare case histories and assess problems
  • Record observations
  • Review reports from health care professionals to determine patients’ current and potential functioning levels
  • Perform administrative tasks
  • Maintain accurate records
  • Liaise with community agencies or partners, and identify additional or alternative services and provide referrals
  • Be the patient advocate
  • Analyse clients' capabilities and expectations related to life activities through observation, interviews and formal assessments
  • Provide end-of-life counselling and bereavement support to patients and their families

Credentials

Certificates, licences, memberships, and courses 

  • Accreditation or registration with the appropriate national, provincial or territorial professional association
