JOB SUMMARY
The Moments Coordinator manages the overall operations of the Moments Neighborhood and has primary responsibility for meeting the daily living activities and life enrichment needs of the Moments Neighborhood residents. The coordinator has direct supervisory responsibility for the Moments Program Resident Specialists and is a champion and resource in the Community for all elements and components of the Moments Program.

ESSENTIAL JOB FUNCTIONS
The Moments Program Coordinator directs the ongoing operation of the Moments Neighborhood through the completion of the following duties:
Supervisory
1. Ensures staff are trained and understand the Essence of Symphony, our Mission and Core Values.
2. Develops and maintains the RS work routines, balancing and reassigning staff as resident census and care needs change. Audits rooms and compliance to work routines daily. Walk and Watch.
3. With the Director of Care (DOC) Proactively schedules and reschedules staffing of the Moments Neighborhood based upon the occupancy and specific care needs of the current residents so as to ensure appropriate levels of staffing, while managing within the budgeted guidelines
4. Trains and supervises care staff and volunteers assigned to the Moments Neighborhood in providing day to day resident care in compliance with approved SSL programs, policies and procedures.
5. Delivers Moments training and orientation programs to all new Community employees and ensures all Moments Neighborhood staff are trained and provide care and life enrichment according to the prescribed Program elements.
6. Models validation and “making connections” techniques while assisting/caring for Moments residents
7. Does final interviews and contributes to hiring decisions with the DOC, CEO and other resources.
8. Orients new staff in collaboration with the DOC and other members of the Leadership Team.
9. With the DOC, conducts probationary and annual performance appraisals on staff working in the neighborhood.
10. Responsible for monitoring and tracking attendance, as well as performance, against established quality standards and for effectively carrying out disciplinary action, as appropriate. Id supported by the DOC in carrying out discipline.
11. Develops and encourages teamwork and promotes positive staff morale. Demonstrates good program execution by lead by example.
12. Provides guidance and acts as a resource to Team Members and Volunteers
Resident Service
1. Conducts family interviews regarding resident life stories to ensure completion of the Resident Profile and the creation of a Moments Memory Box for all new residents to the Neighborhood within 14 days of move in.
2. Coordinates the monthly and daily activities calendars with the Life Enrichment Coordinator ensuring opportunities are provided for Moments residents to participate in activities within other Community Neighborhoods as appropriate and that individual interests and needs are accommodated.
3. Ensures that the Moments Neighborhood maintains the appropriate Life Skill Stations, Moments Kits and Moments Memory boxes and that Team Members are trained in their proper usage with residents.
4. Actively participates in the resident assessment and service planning process, working closely with the DOC and based on the family interviews and ongoing evaluation of resident needs
5. Establishes and maintains regular communication with resident family members and/or POA with at minimum the creation and distribution of the Monthly wellness report. Gets feedback and input from peers to create the report each month for each resident.
6. Participates with the DOC in the development of Moments resident Service Plans (SP).
7. Implements, monitors and updates the SPs ensuring resident care and lifestyle needs are successfully being met
8. Establishes, promotes and facilitates MONTHLY scheduled family educational/support group - in person and online.
9. Works in coordination with members of the Leadership Team to effectively communicate with family members regarding residents' changing needs.
10. Plans family style dining in the neighborhood and hydration requirements with the Dining Services Coordinator
11. Organizes and conducts monthly orientation and in-services on memory care related topics.
12. Evaluates the quality of care delivered to ensure compliance with SSL program policies and procedures
13. Maintains safety and environmental standards for the Moments Neighborhood - Sharps, chemical storage, trip hazards. Ensures common areas and kitchen is clean and well organized.
14. Assists residents with direct care activities of daily living (ADLs) as needed and when staff are not available - with DOC reassess work routines, and assess staff abilities, when this consumes a lot of their time.
Customer Service and Marketing
1. Ensures that all employees provide excellent customer service to internal and external customers.
2. Ensure staff on Moments neighborhood, understand how to appropriately answer the phone, how to handle visitors and how to handle complaints or issues professionally.
3. Ensure all staff are dressed appropriately in Symphony Dress code and name tag, and meeting the standards as outlined in the SSL dress code policy. Ensures staff are not carrying or using personal cell phones while on duty.
4. Participates in the marketing events of the Moments Program and the Community
Other
1. Identifies and implements opportunities to model the Whole Home Approach within the greater Community.
2. Is available for all emergencies of the home, including fire, power outages, elopement, water issues and other emergencies as defined by the CEO.
